Job Overview

A growing home health provider is seeking Physical Therapists (PTs) to join its Skilled Home Health team serving patients throughout Hillsdale, Lenawee, and Monroe counties in Michigan .

In this role, the Physical Therapist will provide in-home therapy services focused on helping patients restore mobility, improve strength, and regain independence following illness, injury, or surgery. The position involves working closely with interdisciplinary care teams to develop and implement individualized treatment plans that support optimal patient outcomes in the home setting.

Key Responsibilities

• Provide physical therapy services to patients in their homes in accordance with physician orders and care plans
• Evaluate patient mobility, strength, balance, and functional abilities
• Develop and implement individualized therapy treatment plans
• Educate patients and caregivers on exercises, mobility techniques, and safety strategies
• Document patient progress and treatment outcomes in accordance with regulatory and agency standards
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Complete required clinical documentation including OASIS assessments when applicable

Qualifications

• Licensed Physical Therapist (PT)
• Current state PT license required
• Home health experience preferred
• Experience with OASIS documentation preferred
• Experience with Homecare Homebase (HCHB) preferred
• Strong clinical assessment and patient education skills
• Ability to work independently while managing a patient caseload

Orientation and Training

New hires will complete a two-day branch orientation followed by field precepting , with training length based on experience.

• Full-time position (40 hours per week)
• Day schedule with weekend rotation
• Territory coverage across Hillsdale, Lenawee, and Monroe counties
